What You Will Do

Practice independently in the community as part of an interdisciplinary care team.
Conduct comprehensive patient assessments and develop individualized treatment plans.
Deliver personalized home-based physical therapy services.
Monitor patient progress and adjust plans of care as appropriate.
Educate patients and caregivers to support recovery and independence.
Collaborate with physicians and interdisciplinary team members.
Manage your own flexible schedule while meeting Guaranteed Flex Pay role expectations.
Utilize innovative technology to efficiently document patient care.

Qualifications

Minimum of one (1) year of Physical Therapy experience preferred, or successful completion of the VNS Health Rehab Intern program.
Current New York State Physical Therapist License and Registration required.
Valid driver's license may be required based on assigned territory.

Pay Range per Visit:

Bronx & Westchester: Weekday visits - $105 | Weekend visits - $110
