Start of Care Nurse (RN), FT3, Home Care

What You Will Do

Practice independently in the community as part of an interdisciplinary care team.

Deliver personalized nursing and care management to patients in their home or care facilities.

Constantly evaluate evolving patient needs and respond with plan of care adjustments.

Qualifications
Licenses and Certifications:

BLS certification preferredCurrent New York State Registered Nurse license and registration required

Education:

Associate's Degree in Nursing required Bachelor's Degree in Nursing from an accredited nursing program preferred

Work Experience:

Minimum one year of recent clinical nursing experience in home care, medical-surgical, acute care, or related setting. required Experience completing OASIS assessments and Start of Care visits preferred. preferred Demonstrated knowledge of home care regulations and reimbursement practices preferred. preferred Strong computer and electronic medical record (EMR) skills required. required Valid driver’s license or government-issued photo ID required based on operational needs. preferred
