MDS Assessor

The MDS Assessor will be responsible for completing accurate and timely resident assessments, supporting individualized care planning, and ensuring compliance with federal and state regulations.

Responsibilities:

Complete MDS assessments accurately and within required timeframes

Review resident documentation and clinical records

Coordinate with nursing, therapy, social services, dietary, and other departments

Participate in interdisciplinary care-plan meetings

Ensure assessments reflect each resident’s condition and level of care

Monitor documentation for accuracy, consistency, and compliance

Support proper reimbursement under the Patient-Driven Payment Model (PDPM)

Maintain compliance with CMS and New York State requirements

Requirements:

Active New York State RN license

Previous MDS experience in a skilled nursing or long-term-care facility required

Strong understanding of MDS 3.0, PDPM, and care planning

Excellent organizational and documentation skills

Ability to meet deadlines and work collaboratively with an interdisciplinary team
