Job Summary: The Intake Coordinator assumes day-to-day responsibility for ensuring excellent customer service by coordinating patient referrals for home care services. This role is responsible for promoting the smooth flow and continuity of patient care within the department by facilitating timely referrals, maintaining clear communication among providers, patients, and families, and coordinating services to support safe and effective transitions of care.

Key Responsibilities:

Answers incoming calls, gathers referral and intake information, and enters all data into the EHR system.

Completes intake documentation, including ICD‑10 coding, and routes all materials for admission approval.

Obtains insurance verification for new referrals.

Conducts initial nursing insurance authorizations and maintains an up‑to‑date authorization tracking log within the EHR.

Maintains a weekly referral log for all incoming intakes.

Collaborates effectively with supervisors, patients and families, referral sources, and clinical staff.

Escalates complex or challenging cases to the Intake Manager.

Assists with staff in‑services and educational opportunities as needed.

Builds and maintains positive working relationships with staff and community partners involved in home‑care services.

Provides support to other branch locations when necessary.

Performs additional duties as assigned by the Intake Supervisor, Manager, or Administrator.

Functional Abilities:

Must be able to remain in a stationary position (e.g., seated or standing) for extended periods while performing desk-based tasks.

Must be able to operate a computer and other standard office equipment (e.g., copier, scanner, phone system).

Must be able to communicate effectively in English with internal and external contacts in both verbal and written form.

Must be able to read and understand in English written documents and data on a computer screen and in printed form (font size 12 or larger).

Must be able to exchange accurate information in one-on-one and group settings.

Must be able to occasionally lift or move office supplies and materials weighing up to 20 pounds.

Must be able to perform multiple tasks and manage frequent interruptions in a fast-paced office environment.

Must be able to travel within the office or between departments as necessary to complete tasks.

Qualifications:

Must be able to read, write and maintain records in English

Must be customer services oriented, detail oriented and an above average communication ability.

Must be organized and have the ability to be multi-tasked with excellent customer service skills.
