A rehabilitation and skilled nursing facility in Brooklyn is seeking a Social Worker (SW) to join its care team.
*Salary:* 65k-75k
*Duties:*
Maintain and/or improve residents’ ability to manage physical, mental, and psychosocial needs
Identify individual social and emotional needs
Develop and maintain individualized care plans
Participate in admission planning, community re-entry, and transfers
Attend interdisciplinary care plan meetings to support residents’ emotional and social well-being

*Requirements:*
Master of Social Work (MSW)
Long-term care or subacute experience required
