Position Overview: The Chief of Care Management provides executive leadership and strategic direction for the care management workforce across the organization. This role champions a culture of accountability, person-centered care, and operational excellence through strategic oversight, data-driven evaluation, and continuous improvement, while maintaining a direct link to the VP’s to support alignment and prevent gaps between leadership and frontline operations and maintain compliance with OPWDD and other applicable regulations.
Essential Responsibilities:
· Maintains professional standards while fostering a positive and supportive work environment that promotes a healthy balance, encourages growth, recognizes achievements, and prioritizes open communication to help staff thrive and excel.
· Lead and oversee the development and implementation of strategies to strengthen the care management workforce and promote person-centered care.
· Provide high level oversight to the VPs of Care Management and Care Management workforce, ensuring clarity of expectations and professional growth of the workforce.
· Foster a culture of accountability, transparency, and high performance across the care management workforce.
· Continuously monitor and leverage data and analytics to monitor care management outcomes, identify trends, inform strategic and operational decisions, and drive accountability across regions.
· Collaborate with leadership to ensure alignment and consistency of care management teams with company-wide objectives and expectations.
· Proactively identify opportunities for improvement and standardized processes within Care Management and drives problem-solving initiatives from identification through resolution.
· Support alignment and coordination between care management and other relevant departments to drive system-wide improvements.
· Serve as a strategic liaison with OPWDD and other relevant agencies to address escalated issues.
· Attend high level agency meetings, such as the Executive Oversight Committee and Strategic Initiative meetings.
· Ensure timely responses to all inquiries and requests, with a commitment to addressing and resolving issues within 24 hours to maintain efficient operations and employee/individual satisfaction.
· Take lead and ownership of assigned tasks and special projects within the teams and assist with coverage as needed.
· Ensures compliance with all OPWDD regulations as well as Tri-County Care policies.
· Participation in internal and external meetings and committees as needed.
· Performs other related duties as assigned by supervisor, to support overall organization’s goals.

Required Experience, Minimum Qualifications
· Master’s degree in health administration, Human Services, Social Work, Nursing, or a related field, preferred
· Minimum of 10 years related Human Service experience, of which 5 is IDD.
· Three years of previous experience at a management level.
