Overview:

Westchester Jewish Community Services (WJCS) is an award-winning, nonprofit, nonsectarian human services organization founded in 1943. Today, WJCS is the largest provider of licensed outpatient community behavioral health services and one of the largest human service agencies in Westchester County. Through innovative programs and compassionate care, we serve more than 20,000 individuals of all ages and backgrounds each year.


We are currently seeking a Residential Documentation & Program Support Coordinator/Qualified Intellectual Disabilities Professional (QIDP) to join our Community Programs team. In this role, you will be responsible for ensuring the quality, accuracy, and regulatory compliance of residential and community-based service documentation; following all applicable policies and procedures; maintaining contact with families and outside agencies; and carrying out responsibilities associated with ISS funding and the Community Habilitation program.


The salary range for this full-time position is $50,000 - $59,000.


Responsibilities:

Documentation & Compliance responsibilities include:

  • Serve as the Qualified Intellectual Disabilities Professional (QIDP) 
  • Review, monitor, and certify residential documentation for quality, completeness, and regulatory compliance. 
  • Ensure timely collection and completion of required documentation and service records. 
  • Conduct documentation audits and support quality assurance, audits and compliance reviews. 
  • Maintain accurate records, tracking systems and reporting requirements.


Community Habilitation responsibilities include

    • Oversee Community Habilitation services for assigned participants. 
    • Develop, review, certify, and update Staff Action Plans for Community Habilitation services, as needed.
    • Attend Life Plan meetings for assigned Community Habilitation participants; review and certify monthly documentation. 
    • Monitor documentation for quality, accuracy, completeness, and compliance.


Housing Support responsibilities include:

    • Maintain monthly contact with individuals receiving housing subsidies and/or designated collateral contacts. 
    • Monitor housing stability and identify, address and document related issues.
    • Coordinate with Care Managers, families, housing providers, and service team members as needed.
    • Responsible for maintaining full occupancy of all apartments by conducting outreach, screening prospective tenants, and coordinating the intake and paperwork process. 
    • Follow up on any issues related to rent or issues with housing/building.
    • Maintains regular contact with building management.



Requirements:

We are seeking candidates who possess the following qualifications:

    • BA or BS Degree in Human Services, Psychology, Social Work, Special Education, Rehabilitation Counseling, or related human service field. 
    • Minimum of one year of experience working with individuals with intellectual and developmental disabilities.
    • Must meet qualifications to serve as a Qualified Intellectual Disabilities Professional (QIDP).
    • Demonstrated experience with treatment planning, and habilitation goal development. 
    • Skilled in utilizing Microsoft Office Suite (Word, Excel, Outlook, Teams) and navigating electronic health record systems.
    • Excellent organizational skills and attention to detail.
    • Strong written and verbal communication skills. 
    • Able to remain awake, alert, and fit for duty throughout scheduled work hours
    • Valid New York State Driver's License and ability to travel to program sites as needed.
