Job Summary

Carease Health is seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner with our organization, you will have the opportunity to work in a hybrid role that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

About the Role

As a key member of our clinical team, you will be responsible for building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player. With a focus on reducing hospitalizations and improving patient outcomes, our Nurse Practitioner will provide high-quality care to residents with chronic conditions, using our innovative remote patient monitoring, chronic care management, and collaborative care management programs.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team

• Build strong relationships with facility staff and residents

• Maintain a patient-centered approach and provide collaborative care

• Utilize remote patient monitoring, chronic care management, and collaborative care management programs to reduce hospitalizations and improve patient outcomes

• Stay up-to-date with the latest clinical guidelines and research to provide best-practice care

Requirements

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and Documentation review

• Excellent communication, collaboration, and teamwork skills

• Ability to work effectively in a fast-paced environment
