Job description

Key Responsibilities:

Conduct inpatient consults for new cancer or blood disorder cases
Evaluate and manage new patients admitted to the hospital
Follow up with established inpatients to ensure continuity of care
Act as a liaison between inpatient and outpatient teams to ensure seamless transitions and care coordination
Proactively alert outpatient teams for follow-up, enhancing care continuity and patient experience
Schedule: Days, Monday – Friday (5) 8hr weekday. This job excludes weekends, after-hour or call responsibilities.

Experienced APPs in this field is a plus!

Qualifications:

Completion and graduation from an accredited Nurse Practitioner or Physician Assistant Program
Minimum 2 years APP experience
CPR certification
Eligible for Maryland and DC licenses
Oncology experience preferred but not required
