At Carease Health we're dedicated to providing exceptional care and support to our patients, and we're seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team. As a Social Work/Care Companion, you'll play a vital role in providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.
About the Role
This part-time position is ideal for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment. As a Social Work/Care Companion, you'll work closely with our experienced clinical team, conducting assessments, implementing short-term therapy, and documenting visit notes and records.
Key Responsibilities
• Visit designated facilities in accordance with the scheduled set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Qualifications
• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Education: Associate Degree
• Experience: 1 year
