Job description
The CHAI Works Program Coordinator works with individuals with mild to moderate developmental disabilities and psychiatric needs and assists participants in developing volunteer and work skills in an integrated setting.
Essential Job Functions:
- Ensures safety and provides comprehensive assistance to participants 5 days a week including integrating adults into the community & participating with individuals during internal and community-based activities/groups.
- Independently supervises participants at the JF&CS location and in the community.
- Ensures programming begins as soon as participants arrive. Continue programming until participants depart for the day, minimizing downtime.
- Acts as a role model to individuals by displaying proper dress, language, and character skills.
- Instructs, supervises, and provides supports to participants, in groups and/or 1:1, regarding: behavior management, personal development, social skills, work attitude, work skills, personal care, communication, self-advocacy and other areas identified in annual plan.
- Assists participants to achieve goals by maintaining knowledge of their learning styles, strengths and needs and ensure that skills are taught accordingly.
- Ensures volunteer work performed meets sites’ expectations, assisting participants to work efficiently and accurately.
- Ensures that Health and Safety policies and procedures are implemented: be familiar with evacuation routes, ensure safe and efficient evacuations during drills and actual emergencies, keep aisles and means of egress clean and accessible, report unsafe situations, monitor participants at lunch/breaks, safely transport participants to community sites. Keeps work areas clean and organized, performs light housekeeping and trains participants to keep work areas orderly.
- Participates in job development activities for creation of individual placements and volunteer sites.
- Communicates effectively with program staff and individuals associated with participants’ services, in a collaborative team effort.
- Establishes rapport, good working relationships, and appropriate boundaries with participants, stake holders, and colleagues.
- Establishes strong relationships with JF&CS internal staff membership and community partners who provide work and volunteer opportunities for participants.
- Organizes and updates participant records; completes required paperwork in a timely and accurate manner.
- Completes daily ECM notes for each assigned participant for that day.
- Completes monthly progress notes, annual assessments, vocational exploration assessments, and other required documents/assessments, including creating individualized service plan (ISP) measurable goals and implementation strategies for all participants on assigned case load.
- Ensures the implementation of participants’ ISP objectives and corresponding data collection; accurately documents progress and needs related to goals. Trains other program staff re: ISP implementation
- Supports participants’ choices in program planning. Participates and negotiates in team meetings and support planning. Act as a resource to participants, families, referral and residential staff. Maintains positive relationship with referral agencies.
- Transports participants in program vans and personal vehicles, as assigned.
- Picks up/drops off vans located off-site, as assigned.
- Works with participants in groups that do not meet at JF&CS/in the community is required.
- Works at other JF&CS day program locations, as assigned
Education and Experience:
- Bachelor’s degree in related field preferred.
- Experience working with individuals with intellectual/developmental disabilities preferred.
Required Skills/Abilities:
- Excellent teamwork skills, the ability to take initiative, and strong interpersonal/communication skills
- Ability to pass First Aid/CPR/Medication Administration Program trainings.
- Must have valid driver’s license, safe driving record, and proof of insurance.
- Familiarity with Microsoft Office including Intermediate Skills in Outlook and Word and basic skills in Excel or willingness to develop such skills within 90 days of hire.
- Knowledge and utilization of ECM database on a regular/daily basis, JF&CS will provide training.
Physical Requirements:
- Must be able to lift up to 50 pounds at times.
- May include assisting participants in and out of wheelchairs and vehicles.
