Social Work/Care Companion

As a compassionate and dedicated Mental Health Provider, you'll play a vital role in our Collaborative Care team, providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. If you're passionate about making a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment, this part-time opportunity is perfect for you.

Key Responsibilities

  • Visit designated facilities in accordance with the schedule set with your supervisor

  • Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Document visit notes and maintain detailed records of interactions and interventions after each visit

  • Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications

  • Associate, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • Experience: 1 year
