Integrated Health is seeking compassionate, computer-savvy individuals to join our team as Care Managers. This is a part-time position offering flexible scheduling for the right candidates.

Responsibilities:

• Provide dedicated care management support to individuals with special needs and mental health conditions.
• Serve as a strong advocate for clients to ensure they receive appropriate services and support.
• Maintain accurate and detailed documentation of client interactions, progress, and care plans.
• Utilize computer systems effectively to manage caseloads and administrative tasks.

Requirements:

• Bachelor’s degree or 3+ years of professional experience working with special needs or mental health populations.
• Demonstrated strength in client advocacy.
• Proficiency in computer skills and digital documentation.
