Case Manager

Rachel’s Place is seeking a female Case Manager to provide individualized support and guidance to young women in our program.

Responsibilities

1. Provide individualized case management and ongoing support
2. Develop and monitor individualized goal plans
3. Connect residents with appropriate community resources and services
4. Advocate for residents and help them navigate available support
5. Help residents develop the skills needed for independent living
6. Collaborate with program staff and outside providers to coordinate care and services
7. Maintain accurate documentation and ensure compliance with program standards

Requirements

1. Bachelor’s degree required
2. Case management experience is a plus
3. Strong communication, organizational, and interpersonal skills
4. Ability to work collaboratively with residents, staff, and outside providers
5. Compassionate, responsible, and detail-oriented approach
