Case Management Specialist

Sunray Home Care is seeking a motivated, professional, and outgoing Case Manager to join our growing team. This position may be remote or in-office, depending on experience and qualifications.

We are looking for someone who is an excellent communicator and comfortable speaking with people throughout the day. The ideal candidate combines strong case-management and organizational skills with a sales and customer-service mindset.

Responsibilities

• Communicate professionally with clients, potential clients, referral sources, healthcare providers, and community organizations
• Follow up with leads and individuals interested in available services
• Explain programs and services clearly and confidently
• Build relationships and maintain consistent communication with clients and referral partners
• Conduct screenings, gather required information, and assist individuals throughout the enrollment or service process
• Make outbound calls and follow up consistently until cases are completed
• Maintain accurate documentation and data entry
• Work closely with management and other team members to meet goals
• Identify opportunities to connect eligible individuals with appropriate services

Ideal Candidate

• Exceptional verbal and written communication skills
• Strong sales, persuasion, and relationship-building skills
• Comfortable making a high volume of phone calls and speaking with new people
• Friendly, confident, professional, and persistent
• Strong follow-up skills — does not allow leads or cases to fall through the cracks
• Organized and able to manage multiple cases simultaneously
• Computer literate and comfortable learning new systems
• Able to work independently and be accountable for results
• Previous experience in case management, home care, healthcare, Medicaid programs, customer service, intake, outreach, or sales is a plus
• Bilingual English/Spanish is a major plus
