QUALIFICATIONS
Qualifications & Experience
Required
Doctor of Pharmacy (PharmD) degree, or Bachelor of Science in Pharmacy with relevant hospital pharmacy experience.
Completion of a PGY1 Pharmacy Residency or equivalent acute care hospital experience.
Eligibility for Virginia pharmacist licensure.
Strong clinical knowledge of pharmacotherapy and medication management principles.
Ability to work collaboratively within multidisciplinary healthcare teams.
Preferred
Board Certification (e.g., BCPS or other applicable specialty certification).
Prior experience in acute care or hospital-based clinical pharmacy practice.
Experience participating in clinical initiatives, protocol development, or pharmacy education programs.
Demonstrated excellence in communication, collaboration, and patient-centered care.

KEY RESPONSIBILITIES
Provide direct clinical pharmacy services across multiple patient care areas, including intensive care, medical-surgical units, obstetrics, nursery services, emergency medicine, and ambulatory infusion.
Participate as a member of interdisciplinary care teams to optimize medication therapy and improve patient outcomes.
Review and verify medication orders to ensure safety, appropriateness, and compliance with clinical standards.
Develop, implement, and evaluate medication-related programs, protocols, and clinical initiatives.
Assist in the creation and maintenance of order sets, medication management policies, and formulary strategies.
Conduct medication use evaluations and assess compliance with evidence-based clinical practice guidelines.
Serve on hospital committees and contribute to quality improvement, patient safety, and operational initiatives.
Educate healthcare professionals on medication therapies, best practices, and emerging clinical developments.
Develop and administer competency assessments and training programs for pharmacy staff and clinical personnel.
Precept pharmacy students and support professional development initiatives.
Participate in scheduled weekend clinical coverage as part of the pharmacy team rotation.
