Field RN – Home Care

Location: Brooklyn, NY
Schedule: Full-Time
Compensation: $55–$60/hour + Benefits

A growing home care organization is seeking a Field RN to conduct in-home assessments and develop individualized plans of care for patients throughout Brooklyn.

This is not a skilled nursing position. The role focuses on patient assessments, care planning, documentation, and coordinating services based on patient needs.

Responsibilities:

• Conduct in-home assessments and evaluate patient needs.
• Develop and update individualized plans of care.
• Coordinate appropriate services and follow up on patient needs.
• Maintain accurate clinical documentation.
• Communicate with patients, families, and care teams.

Qualifications:

• Active New York State RN license required.
• Home care experience preferred.
• Strong assessment and documentation skills.
• Excellent communication and organizational abilities.
• Comfortable working independently in the field.
