Field RN – Home Care

Location: Rockland & Orange Counties, NY
Schedule: Full-Time
Compensation: $55–$60/hour + Benefits

A growing home care organization is seeking a Field RN to conduct in-home assessments and develop individualized plans of care for patients throughout Rockland and Orange Counties.

This is not a skilled nursing position. The role focuses primarily on assessments, care planning, documentation, and coordinating appropriate services based on each patient's needs.

Responsibilities:

• Conduct in-home patient assessments.
• Develop and update individualized plans of care.
• Evaluate patient needs and recommend appropriate services.
• Complete required documentation and follow-up.
• Communicate with patients, families, and care teams.
• Ensure care plans remain appropriate as patient needs change.

Qualifications:

• Active New York State RN license required.
• Home care experience preferred.
• Strong assessment, documentation, and communication skills.
• Ability to work independently and travel throughout the assigned territory.
