Medicaid Case Coordinator

We’re looking for a motivated, personable Medicaid Case Coordinator to join our growing team in Lakewood.

This is not a routine administrative position. You’ll be actively communicating with families, nursing facilities, Medicaid offices, and internal teams to keep Medicaid cases moving forward and solve problems when they arise.

Please note: This position is 100% in-office in Lakewood, NJ.

What You’ll Do

• Client & Facility Outreach
• Review facility census reports and identify potential Medicaid cases
• Reach out to clients and families to discuss services and eligibility
• Build strong relationships with nursing facilities, LTC staff, clients, and intake representatives
• Provide a balance of excellent customer service, relationship-building, and sales-oriented communication
• Own Your Medicaid Cases
• Monitor Nursing Home Medicaid cases throughout the submission and approval process
• Stay on top of case status and identify anything holding up an approval
• Troubleshoot issues and work toward getting them resolved quickly
• Communicate regularly with Medicaid offices, facilities, clients, and internal teams
• Make sure documentation and follow-ups are completed accurately and on time
• Keep Cases Moving
• Follow up consistently with county Medicaid offices
• Build strong working relationships with Medicaid caseworkers and supervisors
• Obtain updates and proactively push cases toward approval
• Escalate unresolved issues when necessary while maintaining professional relationships

Experience with Medicaid, healthcare, nursing homes, admissions, eligibility, intake, case management, or client services is a plus.

Work Location

100% in-office in Lakewood, NJ

Candidates must be able to commute to our Lakewood office daily.
