Job description
Key Responsibilities:
Under supervision:
Provides supportive assistance with activities of daily living (ADLs) through modeling, direct assistance, and coaching.
Assists clients in obtaining basic needs.
Assess client emotional well-being, clinical symptomology, behavioral changes and medication effectiveness.
Provides psychosocial education for consumers related to mental illness, symptom management and medication compliance.
Provides crisis intervention as necessary.
Provides rotating 24 hour on call coverage as scheduled.
Maintain relationships with state and local hospitals.
Participate in daily treatment team meetings and weekly treatment team planning meetings.
Requirements
High School Diploma or equivalent
Bachelor’s Degree preferred
One year of experience in the provision of social services for individuals with severe persistent mental illness
Some lived experience
