What you’ll do:

Perform at the top of your license, delivering best-practice, holistic, patient-centered, evidence-based interdisciplinary palliative medicine.

Provide palliative care consultations and follow-up visits across the hospital continuum – from high-acuity ED and ICU encounters to ongoing management on medical, surgical, and specialty units.

Manage common symptoms and clinical needs for patients with advanced illness and chronic disease.

Lead goals-of-care and advance care planning conversations with patients and families, coordinating with partner SNFs and/or hospices if and when appropriate.

Partner with the interdisciplinary team and hospital teams, integrating into hospital workflows.

Support transitions of care, giving discharge planning as much attention as intake as patients move to their next setting.

Complete timely documentation aligned with NJPCS clinical, billing, and quality standards.

What you bring:

Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.

DEA + CDS registration, or ability to obtain by start date.

2+ years of palliative care experience required.

Palliative care certification (e.g., ACHPN / CHPN) preferred.

Strong communication, reliability, clinical judgment, and comfort working in interdisciplinary care settings.

Compensation & per diem support:

Hourly rate: $75–$80/hour.

No per-visit compensation structure.

Malpractice & professional liability coverage for work performed for NJPCS.
