Job description

Position Responsibilities:

Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
Commits to a respectful, just, and supportive environment for individuals and coworkers aligning with the company’s commitment to diversity, equity, and inclusion.
Effectively manage a tiered caseload, while tailoring services to individual needs.
Completes enrollment and eligibility documentation.
Meets with individuals in their homes, physician/provider offices, and other public places in order to conduct assessments and provide services.
Work independently from home, as well as reporting to the office at least once per week but may require more depending on business needs.

Requirements:

Bachelor’s degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master’s degree with 1 year of relevant experience
Relevant experience consists of providing direct care or support, coordination, and/or advocacy for any person(s) on a regular basis. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Coordinator, etc.
Minimum of a Bachelors degree, or a Licensed RN, is required by OPWDD in NYS
Valid driver’s license
