Regional Care Manager

What you’ll do

Conduct outreach, intake, and comprehensive assessments for ECM and CS members
Develop and implement individualized care plans
Coordinate medical, behavioral health, and social services, including:
Housing navigation and tenancy support
Food access and transportation resources
Provide in-person, telephonic, and virtual services based on member needs
Maintain accurate, timely documentation in APF systems
Collaborate with health plans, community partners, and internal multidisciplinary teams
Ensure compliance with program requirements
Support members in achieving long-term stability and self-sufficiency

Qualifications

Required

Minimum 2 years of experience in:
Case management
Care coordination
Social services or related field
Knowledge of:
Medi-Cal / CalAIM programs
Community resources
Social determinants of health
Strong:
Documentation and organizational skills
Communication and relationship-building skills
Access to reliable transportation
