Job description

RESPONSIBILITIES INCLUDE:
Determine clients’ eligibility for assistance
Interview patients or their representatives to identify problems relating to care
Collect information about clients, maintain and enter information into databases
Consult with parents, teachers, and other school personnel to determine causes of problems, such as truancy and misbehavior, and to implement solutions
Refer patients to appropriate health care, community or social services or resources, educational or work programs
Counsel family members of clients or patients
Monitor clients to evaluate the treatment progress
Confer with clients to discuss treatment plans or progress
Modify treatment plans to accommodate client needs
Schedule and confirm appointments for clients, customers, or supervisors
Coordinate communication between patients, family members, medical staff, administrative staff, or regulatory agencies
Complete documentation required by programs or regulations
Maintain case history records and prepare reports
Assist in ad hoc assignments

Proficient computer skills
Outstanding communication skills, both verbal and written
Excellent organizational skills
Excellent time management skills and the ability to prioritize
Possesses strong attention to detail
Ability to multi-task

EDUCATION:
Master’s degree in a mental health, social work, counseling, or related field, or equivalent experience

EXPERIENCE:
3-5 years of relevant work experience preferred

Outstanding communication skills, both verbal and written
Excellent organizational skills
Excellent time management skills and the ability to multitask
Proficient computer skills
Ability/ experience working with sensitive information
Nonprofit experience is a plus
