Job description

RESPONSIBILITIES INCLUDE:
Develop a thorough understanding of Chai Lifeline case management protocols as applied to the West Coast Division.
Assess client needs and create individualized care plans tailored to each family.
Coordinate and manage all aspects of assigned client care, ensuring timely follow-up and high-quality support.
Build and maintain strong, trusting relationships with families in the West Coast community.
Communicate regularly with parents, children, volunteers, schools, synagogues, hospitals, and community partners.
Oversee logistical components of special gifts, services, and support initiatives for families.
Participate in regional programs, home visits, events, and hospital visits to deepen relationships with families.
Maintain accurate and organized documentation, case notes, and records.
Bring creative ideas, resources, and program concepts to strengthen West Coast Division initiatives.
Collaborate closely with the Director of the West Coast Division and the broader Chai Lifeline team to ensure seamless service delivery.
Assist in ad hoc assignments

EDUCATION:
Master’s degree in Social Work (MSW), Marriage and Family Therapy (MFT), Counseling, Psychology, or a related mental health field required

EXPERIENCE:
2+ years of relevant work or volunteer experience in case management, social services, youth or community support, mental health settings, or related fields

Ability to work independently and collaboratively within a team
