Pharmacist – Staff

Responsibilities
Review and monitor patient’s medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient’s medication therapy and document such interventions. Order verification and performing medication profile reviews that should include but not limited to checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency. Prepare, label, and dispense medications for patients in accordance with physicians’ prescriptions; ensure compliance with Federal, State, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records.

Qualifications
Education:

Required: Pharm. D. or BS in Pharmacy
PGY-1 residency or at least 3 years of equivalent experience with BCPS

Experience:

Preferred: Hospital experience

License:

Required: Current registered pharmacist license in the State of Florida

Certifications:

Required: BLS within 30 days of hire (ACLS and/or PALS if code attendance required)
Preferred: Board Certification in Pharmacotherapy (BCPS), CGP, etc.
