Community Engagement Coordinator

What You’ll Do

Key responsibilities include:

Collaborate with school partners and internal teams to identify student and family needs
Plan and coordinate school-based programs, health fairs, screenings, and educational events
Manage logistics for on-site activities, including setup, materials, and workflow coordination
Provide health education and support to students and families
Assist with patient registration, intake, and demographic verification
Schedule appointments and connect students and families to medical, dental, and behavioral health services
Conduct assessments to identify social needs and connect families to resources
Support care coordination, follow-up, and continuity of care
Build and maintain relationships with school staff, families, and community partners
Promote Care Alliance services through outreach, communication, and engagement activities
Represent Care Alliance at school and community events
Document outreach, registration, referrals, and follow-up activities in the electronic health record
Track participation, outcomes, and engagement metrics to support reporting and program improvement

Work Environment

This role includes both office-based work and frequent on-site activities in schools and community settings. Regular travel within the community and between organizational sites is required. Evening and weekend availability may be required based on program needs.

Required Qualifications
Bachelor’s degree in public health, Social Work, Community Development, or a related field
2–3 years of experience in community engagement, outreach, or program coordination
Strong communication and organizational skills
Preferred Qualifications
Community Health Worker certification or willingness to obtain
Experience working in school-based or youth-serving environments
Experience with electronic health records and patient access workflows
