What You’ll Do

Key responsibilities include:

Serve as Care Alliance Health Center’s primary Community Health Worker within assigned schools.
Build positive, trusting relationships with students, families, school staff, and community partners.
Promote awareness of Care Alliance’s medical, dental, behavioral health, vision, and supportive services.
Participate in school events, parent engagement activities, health fairs, wellness initiatives, and community outreach events.
Assist eligible students and families with enrollment into Care Alliance services.
Help students and families access medical, dental, behavioral health, vision, and specialty care services.
Coordinate referrals, appointments, and follow-up care to support continuity of care.
Conduct Social Determinants of Health screenings and assess barriers affecting student health and well-being.
Connect students and families with resources for transportation, food insecurity, housing instability, insurance coverage, financial assistance, and other social needs.
Support attendance initiatives by addressing health-related and social barriers that may contribute to chronic absenteeism.
Deliver age-appropriate health education to students and families.
Promote preventive care, oral health, behavioral health awareness, nutrition, physical activity, and healthy lifestyle behaviors.
Serve as a liaison between Care Alliance, schools, families, and community organizations.
Document outreach, care coordination, screenings, health education, and case management activities in the electronic health record and other required systems.

Work Environment

This position primarily operates within assigned schools and school-based health settings, with regular collaboration with Care Alliance staff and community partners. The role requires frequent interaction with students, families, school personnel, healthcare providers, and community organizations. Occasional evening or weekend participation in school or community events may be required.

Regular travel between assigned schools, Care Alliance Health Center locations, students’ homes when appropriate, and community partner organizations is required.

Required Qualifications

High school diploma or GED with two years of experience in community outreach, healthcare, education, youth services, or social services; or an associate or bachelor’s degree in public health, Social Work, Human Services, Education, or a related field
Community Health Worker certification or willingness to obtain certification within six months of hire
Experience working with children, adolescents, and families
Strong interpersonal, communication, and relationship-building skills
Proficiency with Microsoft Office applications and electronic documentation systems
Ability to maintain confidentiality and work effectively with diverse populations
Valid Ohio driver’s license and reliable transportation

Preferred Qualifications
Experience working in a school, school-based health center, pediatric practice, or youth-serving organization
Experience providing Medicaid-billable Community Health Worker services or care coordination
CPR/First Aid certification
Bilingual skills, including Spanish or other languages reflective of the communities served
Experience working with underserved or culturally diverse populations
