Qualifications:
Education: A minimum of an Associate’s Degree (Bachelors preferred) in a Human Service field or full certification as an Addictions Counselor Experience: Associate’s Degree: Five (5) years; Bachelor’s Degree: Three (3) years; Master’s Degree: Two (2) years Certified Addictions Counselor (CAC): Five (5) years
Your responsibilities in this role may include:
1. Providing case management services, discharge planning and care coordination for individuals transitioning from an American Society of Addiction Medicine (ASAM) Level Residential Substance Use Disorder Treatment (SUD) Program (3.7, 3.5, 3.3, 3.1)
Handle a caseload of clients to aid them in achieving adjustments to specific barriers and situations of a difficult of complex nature by connecting them with the appropriate community resources Duties may include, but not be limited to providing intake services to clients; preparing reports, treatment plans, discharge/ continuing care plans, and keeping necessary records within client database and electronic health records Coordinating the activities of one or more groups of clients Generating referrals and aftercare plans Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving objectives The work will be reviewed by the Program Director, Program Supervisor or Clinical Supervisor through the completion of assignments.
