*$60,000–$65,000 base salary + performance-based bonus opportunity of up to $30,000 annually*

We’re looking for an energetic, motivated Intake Coordinator to join a growing home care organization during an exciting stage of expansion. This is a high-volume, fast-paced role for someone who enjoys talking to people, building relationships, and following through until the job gets done. Success in this role requires outstanding communication skills, persistence, a strong work ethic, coachability, and the drive to achieve results. You’ll play a key role in helping patients and families access care while building relationships with referral sources, guiding prospective clients through the admissions process, and ensuring every opportunity is followed through from initial contact to approval for services. Comprehensive training will be provided—we’re looking for the right attitude and motivation more than prior industry experience.
