Lead Registered Dietitian (RD)

Position Overview: Centers Health Care is looking to hire a Lead Registered Dietitian to join its clinical team at a skilled nursing facility in the Bronx.

Responsibilities include:
- Assess and monitor residents' nutritional status
- Develop and evaluate individualized nutrition care plans
- Address weight loss, skin breakdown, and nutrition support needs
- Monitor meal service and ensure diet modifications are followed
- Collaborate with the interdisciplinary team, including Speech-Language Pathologists
- Educate residents, families, and staff on nutrition and diet modifications
- Conduct nutrition-related audits and ensure regulatory compliance
- Supervise and work closely with the dietary team
- Maintain accurate documentation and update diet orders/menu changes

Requirements:
- Registered Dietitian (RD) credential
- Degree in Nutrition or Dietetics
- Strong leadership and interpersonal skills
- Knowledge of applicable state and federal regulations
- Computer/EMR proficiency
