Care Companion/Social Worker (Part-time)

Join our team at Carease Health - WI, a leader in delivering comprehensive preventative care programs to skilled nursing facilities. As a Care Companion/Social Worker, you will be a vital part of our Collaborative Care team, providing emotional support, brief therapeutic interventions, and mental health assessments to residents in need.

About the Role

As a Care Companion/Social Worker, you will be responsible for:

  • Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Documenting visit notes and maintaining detailed records of interactions and interventions after each visit

  • Collaborating with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Key Qualifications

  • Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • Minimum 1 year of experience in a similar role
