Social Worker/ Care Companion

Framingham, MA

Are you a compassionate and dedicated Mental Health Provider looking to make a direct impact on the lives of older adults? Carease Health - MA is seeking a skilled Social Worker/ Care Companion to join our Collaborative Care team. As a Social Worker/ Care Companion, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

Key Responsibilities

  • Visit designated facilities in accordance with the schedule set with your supervisor

  • Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Document visit notes and maintain detailed records of interactions and interventions after each visit

  • Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications

  • Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • Education: Associate Degree

  • Experience: 1 year
