Responsibilities:

• Provide case management services to individuals with mental health needs.
• Develop and implement individualized service plans.
• Coordinate with other service providers and community resources.
• Monitor client progress and adjust plans as necessary.
• Advocate for clients' needs and rights.

Requirements:

• Bachelor's degree in social work, psychology, or related field.
• Experience in mental health services preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
