As a Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine, you will play a crucial role in our mission to reduce hospitalizations and improve patient care standards.
About the Role
We are seeking a dedicated and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in State College, PA. In this hybrid role, you will spend time on-site at skilled nursing facilities, conducting patient assessments and developing personalized care plans. You will also work remotely from home, supported by a dedicated scribe team for efficient documentation. Your success will depend on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
Responsibilities
Conduct patient assessments and develop personalized care plans in collaboration with healthcare team members
Provide high-quality preventative care to residents in skilled nursing facilities
Document patient information accurately and efficiently using electronic medical records
Participate in care planning meetings and collaborate with facility staff to ensure seamless continuity of care
Stay up-to-date with changing healthcare regulations and best practices
Requirements
Active NP or PA license (in the state of practice)
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
Ability to work independently and as part of a team
