Nurse Practitioner

Job Summary:

Carease Health - PA is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team of dedicated healthcare professionals. As a Nurse Practitioner, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home. Our hybrid model is designed to provide flexibility and work-life balance while supporting your professional growth and development.

Key Responsibilities:

• Provide comprehensive preventative care to residents in skilled nursing facilities, focusing on proactive measures for enhanced physical health and well-being

• Conduct on-site visits and remote monitoring using software and technology to ensure seamless patient care

• Collaborate with healthcare providers and facility staff to develop and implement individualized care plans

• Participate in the Chronic Care Management (CCM) and Collaborative Care Management (CoCM) programs to address mental health and well-being

• Stay up-to-date with industry best practices and maintain accurate and timely documentation

• Build strong relationships with facility staff, patients, and families to ensure patient-centered care

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
