Patient Coordination Team Lead

📍 9–5, In-Person | Crown Heights

What you’ll do:

• Lead, mentor, and support a team of 5–10 Service Coordinators
• ⁠Run meaningful 1:1s that develop performance, confidence, and accountability
• ⁠Oversee timely patient outreach and clean, accurate documentation
• ⁠Monitor patient retention, step in on escalations, and solve problems before they grow
• ⁠Continuously refine workflows to improve efficiency and patient experienceWhat we’re looking for:
