Our client, a physician services organization, is seeking a Clinical Liaison Representative to serve as the primary on-site liaison between physicians and nursing home staff, across a network of skilled nursing and long-term care facilities.This role serves as the owner’s on-site representative for physician services and related coordination, overseeing execution in the field, ensuring clear communication, and supporting effective clinical collaboration between physicians and facility teams.The position involves regular travel between approximately 15–20 facilities and serves as the central point of contact for day-to-day clinical and operational coordination.Responsibilities:Clinical & Operational LiaisonServe as the primary communication link between physicians and nursing home staffEnsure workflows, expectations, and clinical objectives are clearly understood and followedFacilitate timely communication between physicians, nursing staff, and facility leadershipFacility Oversight & Relationship ManagementTravel regularly between skilled nursing and long-term care facilities to oversee day-to-day program executionBuild and maintain strong professional relationships with facility administrators, nursing leadership, and medical providersAct as the go-to point person for questions, issues, and coordination related to physician services Clinical Support & Follow-UpSupport physicians and nursing teams with scheduling coordination, documentation flow, and communication gapsIdentify operational challenges and escalate concerns appropriately, with proposed solutionsMonitor compliance with program guidelines and facility expectationsAssist with reviewing and coordinating clinical documentation as neededHelp ensure accurate and timely documentation flow between physicians, nursing staff, and facilitiesProgram Quality & Issue ResolutionObserve workflow effectiveness and recommend improvementsAddress concerns proactively to maintain positive facility relationshipsEnsure consistent program presence, professionalism, and service quality across all sitesQualificationsExperience in healthcare coordination, clinical liaison work, provider relations, or nursing supportExperience working with physicians and skilled nursing or long-term care facilitiesBackground in care coordination, utilization management, case management, or clinical operationsComfort acting in an owner’s-representative-style role overseeing execution and relationshipsStrong interpersonal and communication skillsAbility to travel frequently between multiple facilities is a MUSTFriendly, approachable, and professional demeanorHighly organized with strong follow-through skillsKey Traits:Builds trust quickly across physicians and nursing home teamsProactive problem solverComfortable being visible and accountable across multiple locationsStrong balance of clinical understanding and operational mindsetLocation: Always on the road visiting facilities in the 5 NYC Boroughs (headquarters is in Brooklyn NY)Salary: $150K
