What You'll Do

As an SCN Screener Navigator, you'll be the first point of contact for individuals seeking support services. You'll help connect people to critical resources that improve their health, stability, and quality of life.

In this role, you will:

• Welcome and support individuals seeking services
• Conduct eligibility screenings and assessments
• Help connect members with resources such as:

Transportation assistance
Food and nutrition programs
Housing support
Healthcare services
Behavioral health resources
• Assist individuals through the enrollment process
• Build relationships with community organizations and healthcare providers
• Follow up with members to ensure they receive the services they need
• Maintain accurate records and documentation
