Job description
Salary: $26.00 - $38.00 per hour. This is a non-exempt, full-time position with benefits. Bilingual proficiency is required. This position integrates intake, case management, housing navigation and payment processing to support homeless individuals with intellectual and developmental disabilities (IDD) in achieving housing stability. The Housing Case Manager serves as a central point of contact for clients, providing assessments, referrals and direct assistance, processing payments on behalf of clients, and delivering trauma-informed, person-centered case management that centers dignity, choice and autonomy. Responsibilities: • Provide housing-focused stabilization case management to help clients secure and maintain permanent housing • Partner with clients on goal setting and acquiring vital documents • Support clients navigating disability determinations and enrolling in the Medicaid SLS waiver and Home and Community-Based Services • Work with the IDD Housing Navigator to access affordable housing options, mediate with landlords and host home providers, and apply for voucher programs • Connect clients with food access, transportation, benefits counseling, employment, recreational and health services • Provide crisis intervention case management; develop, monitor and modify service plans and exit plans • Maintain accurate case records in required CRMs • Practice solidarity with the disability community by promoting self-advocacy and amplifying client voice in housing and service planning decisions
