Job description

JSSA's Mental Health Services department seeks a full-time Community Care Navigator in Fairfax, VA to address economic insecurity within its Behavioral Health Support Services team. Responsibilities: • Triage and assess needs of referred individuals and families; complete engagement and assessment tools • Develop individualized plans of care using community resources and creative interventions • Provide in-depth care navigation within a Systems of Care framework (DC, MD, or VA) • Serve as facilitator, advocate, and liaison; identify and address gaps in services Qualifications: Bachelor's in social work, psychology, or related field; 2–3 years' relevant experience in DC, MD, or VA; experience navigating social-welfare systems; comfort in a virtual environment; 2+ years with an EHR or similar; Microsoft Office proficiency.
