A Long-Term Care (LTC) facility in Williamsburg, Brooklyn is seeking a full-time Primary Care Nurse Practitioner to provide high-quality, patient-centered care to a geriatric population. This role is ideal for an NP experienced in managing chronic and acute conditions in older adults within a structured LTC environment. **Compensation:** $160,000+ annually **Benefits:** Health, dental, vision, malpractice insurance, and 3 weeks PTO **Location:** Long-Term Care facility in Williamsburg, Brooklyn **Schedule:** Full-time, in-person **Key Responsibilities:** • Perform patient histories and physical exams for approximately 25 geriatric patients per day • Diagnose and manage acute and chronic conditions common in older adults • Order and interpret labs, imaging, and diagnostic tests • Prescribe and adjust medications in accordance with NYS regulations • Coordinate care and make referrals to specialists as needed • Document all encounters thoroughly and accurately in the EHR • Collaborate with interdisciplinary teams including nursing, therapy, social work, and administration • Support continuity of care for long-term residents and subacute patients **Qualifications:** • Current Nurse Practitioner license in New York • Board Certified • 1+ years of geriatric NP experience required • Strong clinical assessment and diagnostic skills • Compassionate, patient-focused approach **Pomera Care** is a healthcare recruitment and consulting agency.
