Job description
The Safe at Home (SAH) Case Manager is a non-exempt position responsible for leading the facilitation of team services within the Safe at Home program model. Working in collaboration with the Family Advocate and Counselor, this role builds the natural support of the family—linking clients involved with Florida Child Protective Services to formal (community providers) and informal (family, friends, neighbors, school) resources to ensure children's safety needs are met alongside family-strengthening interventions. Responsibilities: - Conduct home visits with parents, caretakers, at-risk children, their families, and extended support networks - Provide information, referral, and follow-up for community counseling, psychiatric, financial, employment, parenting, and housing services - Create individualized plans and needs assessments, and coordinate and conduct Family Team Meetings within required timeframes - Help families discover and gain access to community resources that meet their needs - Build and maintain collaborative relationships with peers, stakeholders, case management, and providers focused on child safety and family preservation - Maintain accurate records of all activities, including documentation of phone calls, community linkages, and home visits - Provide peer support and mentorship to volunteers and interns Compensation & Benefits: - $52,000 salary - 15 PTO days and 13 paid holidays per year - Medical, dental, and vision insurance - Immediate eligibility for 403(b) Savings Plan with 25% match - Continuous training, professional development, and mileage reimbursement Requirements: - Bachelor's degree in a related field (e.g., Criminology, Psychology, Sociology, Counseling, Education, Human/Child/Family Development, Marriage and Family Therapy, or Nursing) - One year of professional experience working with children and/or families preferred - Must obtain Child Welfare Case Manager Certification within 12 months of completing paid pre-service training - Ability to work a flexible schedule/on-call rotation and travel in the field year-round - Use of a personal vehicle to transport clients; valid driver's license with auto insurance coverage
